Many of the University's quality procedures require completion of a form. Use of forms helps ensure consistency with regulations, completeness of information and accurate record keeping. As there are a large number of forms for different purposes, this section of the Quality site has been divided into five areas. Click below to access:

Programme Design and Modification
Programme Approval Form: QMU (MSWord)
This form is used to propose a new programme delivered by QMU with no input from a collaborative partner organisation. It was introduced in 2010 and replaces the former Statement of Intent, Academic and Business Case forms. Note that it is required only for full awards and not stand alone modules or short programmes for CPD purposes. It is divided into three parts, the first two of which are considered by the Portfolio Development Group and the third by the School Academic Board. Programme Approval Form: collaborative partners (MSWord)
This form is used to propose a new programme delivered wholly or partially by a collaborative partner organisation. Such programmes may lead either to an award from QMU or a joint or dual degree with another organisation that also has degree awarding powers. It was introduced in 2010 and replaces the former Statement of Intent, Academic and Business Case forms. It is divided into three parts, the first two of which are considered by the Portfolio Development Group and the third by the School Academic Board. Programme Specification (MSWord)
A Programme Specification
is required for each programme leading to an award of the University. The Programme Specification is formally approved at the point of validation and reviewed periodically. Programme Specifications are publicly available on the Quality website (link to current Programme Specifications) Note that the current version posted here no longer includes a requirement for a mapping exercise.
Programme Withdrawal Form (MSWord)
The programme withdrawal form is completed by Heads of Division in order to request withdrawal of a programme.
Module Descriptor Form (MSWord)
The module descriptor form includes information about the content
of the module, learning outcomes and experiences and the assessment
pattern. Please refer to the guidelines below when completing the form.
