Many of the University's quality procedures require completion of a form. Use of forms helps ensure consistency with regulations, completeness of information and accurate record keeping. External Panellist Nomination form (MSWord)
This form is used to nominate External Panel members for validation
and review events. Please note the citeria for nomination. For guidance, contact staff of the Quality Enhancement Unit.
Annual Monitoring Report form
The Annual Monitoring template for Programme Leaders is due to be submitted by 30 November 2011. Guidance Notes are available to assist with completion.

Assessment Feedback for Students (MSWord)

Peer Observation form (MSWord)
Peer observation form for academic staff, including record of observation
form to be submitted to the Dean of School.
Extenuating Circumstances form (MSWord)
Form for submitting claim for extenuating circumstances. Note that this form is currently under review (August 2011).
